Your Surgery

You will need to schedule a preoperative physical with your primary care provider within 90 days prior to surgery. Failure to do so will result in the cancellation of your surgery.
If you are returning to The Eye Surgery Center for a second procedure within 90 days of your preoperative physical, you do not need to have another preoperative physical completed, unless you have had changes in your health and/or an Emergency Department visit.
To provide you with excellent care, we ask that you complete your health history through our secure online portal, One Medical Passport.
This software is a secure, online, patient-controlled account that provides medical history information to your healthcare team.
This minimizes long phone interviews and allows for you to update and use your health history for future appointments.
Your surgeon’s office will provide you with more information after you have been scheduled for surgery. ​

You may receive a text or email message about completing your online history through Medical Passport.
Please follow the links in the message to complete, or go directly to www.OneMedicalPassport.com and follow these instructions.
Inform your surgeon’s office immediately if there are any changes in your health, such as a cold, fever, or respiratory issues.
Refrain from eating or drinking anything (including hard candy, throat lozenges and/or chewing gum) after the times provided
to you by our team.
Additionally, follow any specific instructions provided by your surgeon.

Failure to do so may result in the cancellation of your surgery.
Notify your surgeon if you are taking any blood thinning medications or GLP-1 agonist medications.
It is crucial to arrange for a responsible adult to drive you home and stay with you for the first 24 hours following your surgery.
